• Madhyamik Examination passed or equivalent/ Class VIII passed (duly attested).
• 1 (one) recent passport sized colour photograph (200px X 230px up to 50KB)

• AADHAR Card/ EPIC i.e. Voter’s Identity Card/ PAN card/ Passport or any other Identity Card issued by the Government.
